GlossaryIdiopathic

Idiopathic

Updated June 18, 2026

Idiopathic is defined as a disease or condition that arises spontaneously or for which the underlying cause is unknown. The term is derived from Greek, meaning "a disease of its own kind." In veterinary medicine, this is a "diagnosis of exclusion," meaning the veterinarian has run every test to rule out infection, trauma, toxins, or tumors, and no specific trigger can be found.

In the context of Chronic Kidney Disease (CKD), the term is frequently encountered in two major ways. First, the majority of geriatric CKD cases are technically "idiopathic chronic tubulointerstitial nephritis." This means that while we can see the kidneys are scarred and shrinking, the original event that started the damage, perhaps a silent infection years ago or a genetic predisposition is gone and cannot be identified. Second, cats with CKD are prone to Idiopathic Hypercalcemia, a condition where blood calcium levels rise dangerously high without a tumor or parathyroid issue causing it. For the caregiver, the label "idiopathic" can be frustrating, but it shifts the medical focus from trying to "cure" a root cause to managing the symptoms and maintaining quality of life.
